Interaction Summary
Use caution when coadministering clarithromycin (a CYP3A4 inhibitor and substrate) and efavirenz (a CYP3A4 inducer and substrate) because concomitant use may cause elevated efavirenz exposure, decreased clarithromycin exposure, and increased 14-OH-clarithromycin exposure. Thus, clarithromycin efficacy may be impaired based on the type of bacterial infection being treated, risk of clarithromycin toxicity is increased, and risk of CYP3A4 substrate (eg, efavirenz) toxicity is increased. Dosage adjustments and increased monitoring may be necessary. Alternative antibacterial therapy, such as azithromycin, should be considered . Consider an alternative to macrolide antibiotics with concomitant use of efavirenz due to the increased risk of QT interval prolongation, because both clarithromycin and efavirenz prolong the QT interval .

Literature Reports
A) When 11 subjects were coadministered clarithromycin 500 mg twice daily and efavirenz 400 mg daily for 7 days, the clarithromycin Cmax, AUC, and Cmin were decreased by 26%, 39%, and 53%, respectively. The Cmax, AUC, and Cmin of the 14-hydroxy metabolite of clarithromycin increased by 49%, 34%, and 26%, respectively. Efavirenz Cmax increased by 11% while the AUC and Cmin were unchanged. Forty-six percent of the subjects experienced a rash during concurrent therapy .
Clarithromycin Overview

Clarithromycin is used to treat certain bacterial infections, such as pneumonia (a lung infection), bronchitis (infection of the tubes leading to the lungs), and infections of the ears, sinuses, skin, and throat. It also is used to treat and prevent disseminated Mycobacterium avium complex (MAC) infection [a type of lung infection that often affects people with human immunodeficiency virus (HIV)]. It is used in combination with other medications to eliminate H. pylori, a bacterium that causes ulcers. Clarithromycin is in a class of medications called macrolide antibiotics. It works by stopping the growth of bacteria.

Efavirenz Overview

Efavirenz is used along with other medications to treat human immunodeficiency virus (HIV) infection. Efavirenz is in a class of medications called non-nucleoside reverse transcriptase inhibitors (NNRTIs). It works by decreasing the amount of HIV in the blood. Although efavirenz does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other life-style changes may decrease the risk of transmitting (spreading) the HIV virus to other people.
