Increased lithium concentrations and lithium toxicity (weakness, tremor, excessive thirst, confusion)


Interaction Summary

Generally, caution should be used when lithium and diuretics are used concomitantly because sodium loss may reduce the renal clearance of lithium and increase serum lithium levels, which may lead to lithium toxicity. There are no case reports or clinical studies that document an interaction between lithium and amiloride. A similar diuretic, triamterene, has been reported to increase lithium excretion resulting in decreased lithium effectiveness . Sodium loss may reduce the renal clearance of lithium and increase serum lithium levels with an increased risk of lithium toxicity (Prod Info Lithium carbonate, 2002).

How To Manage Interaction

Monitor serum lithium levels within the first five to seven days of adding or discontinuing amiloride and periodically thereafter. Changes in the lithium dose may be required with concomitant amiloride therapy.

  • Lithium is used to treat and prevent episodes of mania (frenzied, abnormally excited mood) in people with bipolar disorder (manic-depressive disorder; a disease that causes episodes of depression, episodes of mania, and other abnormal moods). Lithium is in a class of medications called antimanic agents. It works by decreasing abnormal activity in the brain.

Amiloride Overview

  • Amiloride is usually used in combination with other diuretics ('water pills') to treat high blood pressure and heart failure in patients who have low amounts of potassium in their bodies or for whom low potassium levels in the body could be dangerous. Amiloride is in a class of medications called diuretics. It works by causing the kidneys to get rid of unneeded water and salt from the body into the urine, but reduces the loss of potassium.

  • High blood pressure is a common condition, and when not treated it can cause damage to the brain, heart, blood vessels, kidneys, and other parts of the body. Damage to these organs may cause heart disease, a heart attack, heart failure, stroke, kidney failure, loss of vision, and other problems. In addition to taking medication, making lifestyle changes will also help to control your blood pressure. These changes include eating a diet that is low in fat and salt, maintaining a healthy weight, exercising at least 30 minutes most days, not smoking, and using alcohol in moderation.
