Increased nortriptyline exposure


Interaction Summary

Cinacalcet is partially metabolized by the CYP2D6 isozyme. In CYP2D6 extensive metabolizers, the coadministration of cinacalcet 25 mg or 100 mg, and amitriptyline 50 mg increased the exposure to amitriptyline and its active metabolite, nortriptyline, by approximately 20%. There were no pharmacodynamic effects reported. Therefore, if cinacalcet and nortriptyline are coadministered, dose adjustments to nortriptyline may be necessary. Also, patients may need to be monitored for signs and symptoms of nortriptyline adverse events (dry mouth, sedation, urinary retention, blurred vision).

How To Manage Interaction

If cinacalcet, a strong CYP2D6 inhibitor, is used concomitantly with nortriptyline, dose adjustments of nortriptyline may be required. Also, closely monitor patients for signs and symptoms of nortriptyline adverse events (dry mouth, sedation, urinary retention, blurred vision).

  • Nortriptyline is used to treat depression. Nortriptyline is in a group of medications called tricyclic antidepressants. It works by increasing the amounts of certain natural substances in the brain that are needed to maintain mental balance.

Cinacalcet Overview

  • Cinacalcet is used alone or with other medications to treat secondary hyperparathyroidism (a condition in which the body produces too much parathyroid hormone [a natural substance needed to control the amount of calcium in the blood] which can cause serious problems with the bones, heart, blood vessels, and lungs) in patients with chronic kidney disease (condition in which the kidneys stop working slowly and gradually) who are being treated with dialysis (medical treatment to clean the blood when the kidneys are not working properly). Cinacalcet is also used to treat high levels of calcium in the blood of patients who have parathyroid cancer (cancer of the glands in the neck that make parathyroid hormone). Cinacalcet is in a class of medications called calcimimetics. It works by signaling the body to produce less parathyroid hormone in order to decrease the amount of calcium in the blood.
