Interaction Summary

Three cases of renal dysfunction have been reported with concurrent use of procarbazine and high dose methotrexate (2 g/m(2)). After 14 days of procarbazine therapy methotrexate was infused within 48 hours of procarbazine. Renal function began to decline within 24 hours to 48 hours of methotrexate administration.

How To Manage Interaction

A period of 72 hours between methotrexate and procarbazine administration is suggested to avoid the possibility of renal dysfunction due to this combination therapy.

Literature Reports

A) Combined therapy with oral procarbazine 100 mg/m(2) daily and intravenous methotrexate 2 g/m(2) daily resulted in impaired renal function for three patients . Renal function was normal prior to drug therapy for all of the patients. Procarbazine was given on days 1 to 14 and the first dose of methotrexate was given within 48 hours after the end of procarbazine therapy. Renal function deteriorated 24 to 48 hours after the methotrexate dose. A delay in administration of the second dose or withdrawal of the methotrexate resulted in normalization of the renal function tests within 3 months. It is unclear from the data presented whether renal impairment occurred secondary to the combination of both agents or from the methotrexate alone and more studies are suggested to better define this interaction. However, as a precautionary measure, a period of 72 hours should elapse between the dosing of these drugs.

Procarbazine Overview

  • Procarbazine is used in combination with other medications to treat certain types of Hodgkins disease (types of cancer that begin in a type of white blood cells that normally fights infection). Procarbazine is in a class of medications called alkylating agents. It works by slowing or stopping the growth of cancer cells in your body.

Methotrexate Overview

  • Methotrexate is used to treat severe psoriasis (a skin disease in which red, scaly patches form on some areas of the body) that cannot be controlled by other treatments. Methotrexate is also used along with rest, physical therapy, and sometimes other medications to treat severe active rheumatoid arthritis (RA; a condition in which the body attacks its own joints, causing pain, swelling, and loss of function) that cannot be controlled by certain other medications. Methotrexate is also used to treat certain types of cancer including cancers that begin in the tissues that form around a fertilized egg in the uterus, breast cancer, lung cancer, certain cancers of the head and neck, certain types of lymphoma, and leukemia (cancer that begins in the white blood cells). Methotrexate is in a class of medications called antimetabolites. Methotrexate treats cancer by slowing the growth of cancer cells. Methotrexate treats psoriasis by slowing the growth of skin cells to stop scales from forming. Methotrexate may treat rheumatoid arthritis by decreasing the activity of the immune system.
