Ethacrynic acid has been reported to enhance anticoagulant activity by displacing anticoagulants from protein binding sites. The risk of gastrointestinal bleeding may also be increased by ethacrynic acid administered with anticoagulants .

How To Manage Interaction
In patients receiving oral anticoagulant therapy with warfarin, the prothrombin time ratio or international normalized ratio (INR) should be closely monitored with the addition and withdrawal of treatment with ethacrynic acid, and should be reassessed periodically during concurrent therapy. Adjustments of the warfarin dose may be necessary in order to maintain the desired level of anticoagulation.

A) A 38-year-old female who was treated with warfarin and ethacrynic acid in varying doses for possible recurrent pulmonary emboli developed prothrombin times as high as 38 to 39 seconds following concomitant warfarin and oral ethacrynic acid administration. Prothrombin time values following warfarin alone ranged from 17 to 23 seconds versus a range of 25 to 39 seconds following concomitant administration .

Warfarin is used to prevent blood clots from forming or growing larger in your blood and blood vessels. It is prescribed for people with certain types of irregular heartbeat, people with prosthetic (replacement or mechanical) heart valves, and people who have suffered a heart attack. Warfarin is also used to treat or prevent venous thrombosis (swelling and blood clot in a vein) and pulmonary embolism (a blood clot in the lung). Warfarin is in a class of medications called anticoagulants ('blood thinners'). It works by decreasing the clotting ability of the blood.
Ethacrynic Acid Overview

Ethacrynic acid is used to treat edema (fluid retention; excess fluid held in body tissues) in adults and children caused by medical problems such as cancer, heart, kidney, or liver disease. Ethacrynic acid is in a class of medications called diuretics ('water pills'). It works by causing the kidneys to get rid of unneeded water and salt from the body into the urine.
