Ashwagandha and Testosterone Undecanoate

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Ashwagandha interacts with whole categories of medication. Testosterone Undecanoate falls into 3 of those categories, so it can interact with Ashwagandha in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

THYROID HORMONE

1 of 3

Why this applies to Testosterone Undecanoate

Testosterone Undecanoate falls under THYROID HORMONE — the group Ashwagandha interacts with.

Ashwagandha might increase the effects and adverse effects of thyroid hormone.
Concomitant use of ashwagandha with thyroid hormones may cause additive therapeutic and adverse effects. Preliminary clinical research and animal studies suggest that ashwagandha boosts thyroid hormone synthesis and secretion. In one clinical study, ashwagandha increased triiodothyronine (T3) and thyroxine (T4) levels by 41.5% and 19.6%, respectively, and reduced serum TSH levels by 17.4% from baseline in adults with subclinical hypothyroidism.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's something worth clearing up: the interaction on file is really about ashwagandha and thyroid hormone, not testosterone. Ashwagandha may nudge your thyroid levels up a bit, but testosterone undecanoate is an androgen used for low testosterone, so it works through a completely different system. The thyroid-related warning doesn't directly apply to this medication.

That said, I can't promise a meaningful testosterone-specific interaction either way, since there isn't solid data on the two together. To stay safe, mention to your pharmacist or doctor that you're considering ashwagandha, especially if you also have any thyroid concerns. They can keep an eye on the right lab work for you.

HEPATOTOXIC DRUGS

2 of 3

Testosterone Undecanoate also falls under HEPATOTOXIC DRUGS.

Theoretically, taking ashwagandha with hepatotoxic drugs might increase the risk of liver damage.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the concern with pairing these two. Oral testosterone undecanoate (like Jatenzo or Kyzatrex) can put some stress on your liver, and your doctor likely already monitors your liver tests while you're on it. Ashwagandha has been tied to rare but real cases of liver injury on its own. So stacking a liver-stressing supplement on top of a medication that's already easy on the liver isn't ideal, even though this is a theoretical worry based on how each one behaves rather than proven cases of the two together.

Watch for warning signs like yellowing skin or eyes, dark urine, belly pain, unusual tiredness, or nausea. This isn't a reason to panic, but please loop in your pharmacist or doctor before adding ashwagandha so they can keep an eye on your liver numbers.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Testosterone Undecanoate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for testosterone undecanoate. Lab studies hint that ashwagandha may speed up an enzyme in your body called CYP3A4, which is one of the systems that helps break down testosterone. If that happened, it could in theory lower your testosterone levels a bit and make the medicine work less well, meaning the low-testosterone symptoms you're treating (low energy, low sex drive, mood changes) might not improve as much.

That said, I want to keep this in perspective. This is rated a minor interaction, and the only evidence comes from test-tube and animal research, not real people. So it's more of a theoretical caution than a proven problem. Since your doctor checks testosterone levels with bloodwork anyway, just mention the ashwagandha to your pharmacist or doctor so they can keep an eye on things.

Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. About Testosterone Injection

What Is Testosterone Injection Used For?

Brand names: Andriol, Aveed, Jatenzo, Kyzatrex

Testosterone injection is primarily used in men whose bodies do not produce enough testosterone on their own — a condition called hypogonadism. This includes primary hypogonadism (a problem with the testicles themselves, such as from injury, infection, or surgery) and hypogonadotropic hypogonadism (caused by problems in the brain's hormone signaling from the hypothalamus or pituitary gland). It is also used to stimulate puberty in carefully selected adolescent males with clearly delayed puberty who have not responded to other support.

Testosterone Enanthate Injection is also approved for use in women with advanced, inoperable metastatic breast cancer (specifically skeletal metastases) who are one to five years past menopause, or in premenopausal women with hormone-responsive breast cancer who have had their ovaries removed. This use should be directed by an oncologist. Aveed (testosterone undecanoate) is specifically approved for male hypogonadism only.
