Clobazam falls under CNS DEPRESSANTS — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might have additive effects if used with other CNS depressants.
Preliminary clinical research, case reports, and animal studies suggest that high dose cannabidiol has sedative and hypnotic effects. Theoretically, concomitant use of cannabidiol with drugs with sedative and anesthetic properties may cause additive therapeutic and adverse effects.

Here's something worth knowing about pairing CBD with clobazam (Onfi, Sympazan). There are actually two things going on. First, the one in our database: both CBD and clobazam can slow down the central nervous system, so taking them together may stack those effects. That can mean more drowsiness, dizziness, trouble concentrating, or feeling unsteady on your feet.

On top of that, CBD is well known to raise levels of clobazam's active form in the body, which can make these sedative effects even stronger. This isn't a reason to panic, and people use this combo under medical care, but it's not one to start on your own. Cannabidiol might increase levels of clobazam and increase the occurrence of somnolence.
In clinical studies, concomitant administration of cannabidiol and clobazam is associated with up to a 60% increase in serum levels of N-desmethylclobazam, the primary active metabolite of clobazam. This increased concentration is likely due to inhibition of CYP2C19 by cannabidiol. However, the interaction does not appear to be dose-dependent. In children and adults, concomitant use of cannabidiol and clobazam is associated with an increased occurrence of somnolence.

Here's the deal with these two together. CBD slows down a liver enzyme called CYP2C19, which is the same enzyme that helps clear out clobazam's active leftover, called N-desmethylclobazam. When CBD blocks that pathway, this active piece can climb by as much as 60 percent, so you're effectively getting more drug effect even without changing your clobazam dose.

What does that look like in real life? Mostly more drowsiness and sleepiness, and sometimes more sedation, dizziness, or trouble with balance. This isn't a maybe, it showed up in solid clinical trials, so it's worth taking seriously. The good news is that doctors often manage it simply by adjusting the clobazam dose. Please don't change anything on your own. Loop in your prescriber or pharmacist first so they can watch your dose and how you're feeling.

Clobazam is used as an add-on (adjunctive) treatment for seizures linked to Lennox-Gastaut Syndrome (LGS), a severe form of childhood-onset epilepsy. It is approved for patients 2 years of age and older.

This indication applies to all available forms: Sympazan (dissolving oral film), Onfi (tablets and oral suspension), and generic clobazam tablets and oral suspension.
