A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of CYP1A2.

Here's the piece of this combo pill that actually matters for grape: the caffeine. Caffeine is cleared by an enzyme called CYP1A2, and a small study suggested that grape juice may speed that enzyme up a bit. In plain terms, your body could clear caffeine a little faster, so the gentle pick-me-up and pain-boosting effect of the caffeine portion might feel slightly weaker. The aspirin and dihydrocodeine parts aren't really affected by this.
Honestly, this is theoretical and based on weak evidence, so I wouldn't lose sleep over it. Eating grapes won't matter, and even concentrated supplements are unlikely to cause a big problem here. If you take grape seed extract regularly, just mention it to your pharmacist or doctor.

Here's the wrinkle with this combo: the dihydrocodeine part of Synalgos-DC is a prodrug. Your body uses an enzyme called CYP2D6 to convert it into its stronger active form, which is what gives you real pain relief. Lab studies hint that concentrated grape seed extract might block that same enzyme. If that happened in people, you could get less pain relief than expected, not more.
The honest part: this is only test-tube evidence and has never actually been seen in humans, so I wouldn't lose sleep over it. Eating grapes won't matter at all. If you're using a concentrated grape seed supplement and notice your medicine isn't working as well, just check in with your pharmacist or doctor first.

In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the honest picture with Synalgos DC (aspirin, caffeine, and dihydrocodeine) and grape seed extract. The dihydrocodeine piece is a bit of a prodrug situation: your liver uses an enzyme system to turn it into its more active painkilling form. Grape compounds might nudge that enzyme either way. In a lab dish they seem to slow it down, but in animal studies they appear to speed it up. So we honestly can't say which direction it goes in people.
This is only test-tube and animal data, and it has never actually been seen in humans, so I wouldn't lose sleep over it. In theory, more activation could mean extra drowsiness or constipation, while less could mean weaker pain relief. Eating grapes is fine. Before adding a concentrated grape seed supplement, just run it by your pharmacist or doctor.
