Losartan Potassium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase or decrease levels of drugs metabolized by CYP2C9.
Animal research shows that gymnema can induce the CYP2C9 enzyme. In one animal study, gymnema caused a 2.4-fold increase in the clearance of tolbutamide, a CYP2C9 substrate, in rats. In vitro research also shows that gymnema can inhibit CYP2C9.

Here's something worth knowing about pairing Gymnema with losartan (Cozaar). Losartan is actually a prodrug, meaning your liver has to convert it (largely through an enzyme called CYP2C9) into its active form that does most of the blood-pressure-lowering work. Gymnema may bump into that same enzyme, but the lab and animal data point in two directions at once: in some studies it speeds the enzyme up, in others it slows it down.

So in plain terms, the effect is hard to predict. If Gymnema revs up that enzyme, you could form a bit more active losartan and see slightly stronger effects (lower blood pressure, lightheadedness). If it blocks the enzyme, losartan may not activate as well and your blood pressure could creep up. Keep in mind this is theoretical, based only on lab and animal research, so I wouldn't panic. Just check in with your pharmacist or doctor, and if you do try Gymnema, watch your blood pressure readings.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the honest picture with Gymnema and losartan (Cozaar). The concern on file is that Gymnema might slow down a liver enzyme called CYP3A4, which helps process losartan. If that enzyme were blocked, a bit more drug could hang around, in theory nudging you toward things like dizziness or lightheadedness from blood pressure dropping too low.

But I want to keep this in perspective. The worry comes from a single test-tube study using rat liver. Other lab and animal research found no effect on this enzyme, and a rat study showed Gymnema didn't change levels of a similar blood pressure drug. So this is a weak, theoretical flag, not something we usually see in real life. Since both Gymnema and losartan can lower numbers (sugar and blood pressure), just give your pharmacist a heads-up before combining them.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
