Amiloride, Hydrochlorothiazide falls under DIURETIC DRUGS — the group Horsetail interacts with.

Theoretically, taking horsetail with diuretic drugs might increase potassium loss and the risk of hypokalemia.
Laboratory research shows that various species of horsetail have diuretic properties. Due to its diuretic effects, there has been concern that taking horsetail along with potassium-depleting diuretics might increase the risk for hypokalemia. However, pharmacokinetic research in humans shows that taking horsetail 900 mg daily for 4 days does not affect urinary excretion of electrolytes, including potassium and sodium, despite having a diuretic effect similar to taking hydrochlorothiazide 25 mg daily. It is unclear if taking horsetail for a longer duration would affect electrolyte levels. Until more is known, use with caution.

Here's the thing about your Moduretic: it's actually two water pills in one. The hydrochlorothiazide part can lower your potassium, while the amiloride part is added specifically to hold onto potassium and balance that out. Horsetail is a mild diuretic too, so on paper, stacking it on top of your medication could nudge you toward losing more fluid and minerals than intended.

That said, this concern is theoretical. A short human study found horsetail didn't actually change potassium or sodium in the urine, so it's far from a proven problem. Still, because horsetail can also deplete thiamine with long-term use, I'd keep your doctor or pharmacist in the loop before combining them, and don't change your blood pressure regimen on your own.

What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
