Ergotamine, Belladonna, Phenobarbital falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

The piece of this combination pill that really matters here is the phenobarbital, which is a barbiturate that calms the brain and nervous system. Kava works in a similar quieting way on the brain, so pairing the two can stack their calming effects on top of each other. This isn't about one speeding up or slowing down the other in the liver, it's simply two sedating substances adding together.

In plain terms, you could end up more drowsy than expected, with slowed reflexes, dizziness, foggy thinking, or trouble staying alert to drive. Major expert guidelines specifically warn against mixing kava with this kind of medicine. There's also a liver concern, since both kava and these drugs can stress the liver. Please don't stop or change anything on your own. Check with your pharmacist or doctor first.

Ergotamine, Belladonna, Phenobarbital also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

This combination product mixes ergotamine (for migraines), belladonna, and phenobarbital (a sedative). The theory on file is that kava might block CYP3A4, a liver enzyme that helps clear the ergotamine portion. If that were true, ergotamine could build up and raise the risk of its known problems, like nausea, tingling or cold hands and feet, or in rare cases reduced blood flow.

Here's the reassuring part: the better human research shows kava does not meaningfully affect CYP3A4, so this interaction is rated minor and unlikely. What I'd watch more closely is the overlap in sedation, since both kava and phenobarbital calm the brain and can leave you extra drowsy. Kava also stresses the liver, so please run this past your pharmacist or doctor first.
