Why this applies to Propranolol

Propranolol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Here's the basic idea with kava and your propranolol. Lab studies show kava can slow down an enzyme called CYP2C19, which is one of the pathways your body uses to clear propranolol. If that enzyme runs slower, a bit more propranolol could linger in your system. Since propranolol is active as-is (not a prodrug that needs switching on), more of it could mean a stronger beta-blocker effect, things like a slower heart rate, lower blood pressure, dizziness, or feeling unusually tired.

I want to keep this in perspective. This is theoretical, based only on test-tube research, and hasn't actually been seen in people. Still, kava itself can stress the liver, so it's worth a quick chat with your pharmacist or doctor before combining the two.

Propranolol also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the short version on combining kava with propranolol: the concern on file is whether kava blocks a liver enzyme called CYP1A2, which is one of the pathways that helps clear propranolol. If kava slowed that enzyme, propranolol could build up a bit and you might notice more of its usual effects, like a slower heart rate, lower blood pressure, lightheadedness, or feeling more tired. Propranolol is an active drug (not a prodrug), so more of it in your system simply means a stronger beta-blocker effect.

The reassuring part: the better human study found kava had no real effect on this enzyme, so this interaction is rated minor and unlikely. A bigger worry is that both kava and propranolol can stress the liver, so please check with your pharmacist or doctor before pairing them.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the short version on combining kava with propranolol. Propranolol is partly cleared by a liver enzyme called CYP2D6. In test-tube studies, kava looked like it might slow that enzyme down, which in theory could let propranolol build up and lead to stronger effects like a slower heart rate, lower blood pressure, lightheadedness, or fatigue.

The good news: when researchers actually tested this in people, kava did not meaningfully change how CYP2D6 medications are handled. So this interaction is rated minor and unlikely. The bigger thing I'd flag is that both kava and propranolol can have calming effects, and kava has been linked to rare liver problems. Please check with your pharmacist or doctor before adding it.

What Is Propranolol (Cardiovascular) Used For?

Brand names: Inderal LA, Inderal, Angilol, Syprol, Inderal Injection, Half-Inderal LA, Bedranol SR, Half Beta Prograne, Beta Prograne, Lopranol LA, Slo-Pro, InnoPran XL

This intravenous (IV) form of propranolol is used in hospital or clinical settings to treat dangerous heart rhythm problems. It is generally reserved for life-threatening arrhythmias (abnormal heart rhythms) or those that occur during anesthesia.

Specific conditions it treats include fast heart rates coming from the upper chambers of the heart (supraventricular tachycardia), certain arrhythmias caused by digitalis toxicity, dangerous ventricular rhythms that haven't responded to other treatments, and fast heart rates caused by excess adrenaline-like substances during surgery.
