Rivaroxaban falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the thing to know if you're taking Xarelto (rivaroxaban) and thinking about phellodendron. Your liver uses an enzyme called CYP3A4 to help clear rivaroxaban out of your body. Phellodendron contains berberine, which in lab and early human studies appears to slow that same enzyme down a bit. If clearance slows, more rivaroxaban could hang around than expected.

Since rivaroxaban is a blood thinner, the realistic concern is more blood thinning, meaning easier bruising, nosebleeds, or harder-to-stop bleeding. To be fair, this is theoretical and based mostly on lab work, not strong real-world proof. Still, because bleeding is serious, please check with your pharmacist or doctor before combining them rather than changing anything on your own.

Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

Here's the simple version: Xarelto (rivaroxaban) is already a powerful blood thinner that lowers your blood's ability to clot. Phellodendron contains a compound called berberine, and in lab and animal studies it appears to make platelets (the cells that help form clots) a little less sticky. Stack those two together and, in theory, you could tip the balance a bit too far toward bleeding.

What that might look like: easy bruising, nosebleeds, bleeding gums, or in rare cases more serious bleeding. I want to be honest, though, this concern comes only from test-tube and animal research, not from people, so it's more of a caution than a proven problem. Still, since rivaroxaban itself raises bleeding risk, I'd check with your pharmacist or doctor before adding phellodendron rather than starting it on your own.

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. Human evidence for most uses is limited, and it is often sold in combination with other herbs, making its individual effects hard to judge. What Is Rivaroxaban Used For?

Brand name: Xarelto

Rivaroxaban tablets are used to reduce the risk of stroke in adults with nonvalvular atrial fibrillation (an irregular heart rhythm), treat deep vein thrombosis (DVT, a blood clot in a vein) and pulmonary embolism (PE, a blood clot in the lungs), and lower the chances of these clots coming back. They also help prevent clots after hip or knee replacement surgery, protect hospitalized patients at risk for clots, and — in combination with aspirin — reduce the risk of major cardiovascular events in adults with coronary artery disease (CAD) or peripheral artery disease (PAD).

Rivaroxaban tablets are also approved for treating VTE (venous thromboembolism, blood clots in veins) and preventing recurrence in pediatric patients from birth to under 18 years, and for preventing clots in children aged 2 and older with congenital heart disease after the Fontan procedure. Rivaroxaban for oral suspension (available as Rivaroxaban Granule and Xarelto oral suspension) is specifically indicated for these two pediatric uses.
