Methotrexate falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Quercetin interacts with.
In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your kidneys help clear methotrexate out of your body using a transporter called OAT3. In lab studies, quercetin blocks that transporter pretty strongly. If that same effect happened in people, your body might hold onto more methotrexate than usual, which could push up its effects and its side effects (think mouth sores, nausea, fatigue, low blood counts, or liver irritation).
I want to be honest about the strength of this, though. So far this is only seen in test-tube research, not in actual patients, so it's theoretical at this point. Methotrexate also has a narrow safety window, so I'd play it safe. Before adding a quercetin supplement, please check with your pharmacist or doctor. Food amounts in fruits and veggies are very unlikely to be a problem.

2 of 2Methotrexate also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).
In vitro evidence shows that quercetin can inhibit organic anion-transporting peptide (OATP) 1B1-mediated uptake of estrone-3-sulfate and pravastatin. Furthermore, clinical research in healthy males shows that intake of quercetin along with pravastatin increases the AUC of pravastatin by 24%, prolongs its half-life by 14%, and decreases its apparent clearance by 18%, suggesting that quercetin modestly inhibits the uptake of pravastatin in hepatic cells.

Here's the gist: methotrexate moves around your body partly through transporter proteins called OATPs. Quercetin, in high supplement doses, can gently slow some of these transporters down. If methotrexate isn't cleared quite as efficiently, more of it could linger in your system, which may nudge up its effects and its side effects, things like mouth sores, nausea, fatigue, low blood counts, or liver irritation.
I want to be honest about the strength of this, though. This concern is mostly theoretical for methotrexate, drawn from lab work and a study with a different drug (pravastatin). The effect there was modest. Still, methotrexate has a narrow safety window, so even small shifts deserve respect. A pinch of quercetin from foods is unlikely to matter, but please run any concentrated supplement by your pharmacist or doctor first.

What Is Methotrexate Used For?

Methotrexate treats a wide range of serious conditions. In cancer care, it is used for acute lymphoblastic leukemia, non-Hodgkin lymphoma, osteosarcoma, breast cancer, head and neck cancer, and gestational trophoblastic neoplasia (a rare tumor related to pregnancy). It is also used to prevent and treat meningeal leukemia (cancer affecting the lining of the brain and spinal cord).
Outside of cancer, methotrexate is prescribed for adults with rheumatoid arthritis (RA), children with polyarticular juvenile idiopathic arthritis (a form of childhood arthritis), and adults with severe psoriasis that hasn't responded well to other treatments.
