Here's the key thing about irinotecan: your body has to convert it into its active cancer-fighting form, called SN-38. That activation, plus how the drug is later cleared, leans on liver enzymes like CYP3A4. St. John's wort speeds up (induces) CYP3A4, which shifts how much active drug you end up with. In real studies, combining the two lowered the levels of the active SN-38, meaning your chemotherapy may simply not work as well as it should against the pancreatic cancer it's fighting.

Because this is a powerful cancer treatment where getting the right exposure really matters, this is not a combination to take lightly. Please don't start or stop anything on your own. Talk with your oncologist or pharmacist before using St. John's wort, since they may want you to avoid it entirely during treatment.

St. John's wort reduces the levels and clinical effects of irinotecan.
St. John's wort 900 mg daily for 18 days decreases serum levels of irinotecan by at least 50%. Clearance of the active metabolite of irinotecan, SN-38, is also increased, resulting in a 42% decrease in the area under the concentration-time curve. This is thought to be due to induction of cytochrome P450 3A4 (CYP3A4) by St. John's wort.

This is a combination I'd really want you to avoid without your oncology team weighing in first. St. John's wort speeds up a liver enzyme called CYP3A4, and that same enzyme is heavily involved in handling irinotecan. In a carefully done study, taking St. John's wort dropped irinotecan blood levels by about half and cleared out SN-38 (the active form that actually fights the cancer) faster too.

The worry here isn't extra side effects, it's the opposite: your chemotherapy could simply work less well against the pancreatic cancer it's meant to treat. That's a serious problem when you're counting on every bit of effect. Please don't start or stop anything on your own. Talk with your cancer doctor or pharmacist before using St. John's wort in any form.

  • Irinotecan lipid complex is used in combination with other medications to treat pancreatic cancer that has spread to other parts of the body that has worsened after treatment with other chemotherapy medications. Irinotecan lipid complex is in a class of antineoplastic medications called topoisomerase I inhibitors. It works by stopping the growth of cancer cells.
