Short-term use of andrographis is generally well tolerated, but long-term safety and the quality of different products aren't well established. Avoid it during pregnancy—animal studies raise concerns about reproductive effects—and don't use it while breastfeeding because there isn't enough safety information.

Common side effects include stomach discomfort, nausea, diarrhea, headache, fatigue, dizziness, altered taste, rash, itching, and hives. Higher doses (5–10 mg/kg of the active constituent andrographolide) and use beyond 14 days increase the likelihood of side effects.

Rarely, severe allergic reactions including anaphylaxis have been reported. Skin reactions—including Stevens-Johnson syndrome (a serious blistering condition), exfoliative dermatitis, and angioedema (facial swelling)—have also been documented.

Theoretically, andrographis might decrease the absorption of etoricoxib, although the clinical significance is unclear.
Animal research shows that andrographis extract, or the constituent andrographolide, taken orally with etoricoxib decreases the bioavailability of etoricoxib. However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
