This is a great question as there are several well-known and potentially dangerous drug interactions between proton pump inhibitors (PPIs), like Nexium, and blood-thinning medications.

One such well-known interaction is between the antiplatelet Plavix (clopidogrel) and Prilosec (omeprazole). Prilosec is an inhibitor of the metabolizing enzyme CYP2C19, which is the same enzyme responsible for metabolizing Plavix into its active form.

The good news regarding Xarelto and Nexium is that there is no evidence to suggest a similar interaction. These two drugs are generally considered safe to take together.

Xarelto is primarily metabolized by several enzymes and transporters, including CYP3A4/5, CYP2J2, P-glycoprotein (P-gp), and ATP-binding cassette G2 (ABCG2).

Nexium does not significantly affect these metabolic pathways or transporters. While it is an inhibitor of CYP2C19, which can increase the exposure of drugs metabolized by CYP2C19, this pathway is not majorly involved in the metabolism of Xarelto.
