Interaction Summary
Concurrent use of carBAMazepine, a potent inducer of CYP3A4 and a CYP3A4 substrate, with cobicistat, a CYP3A substrate and CYP3A inhibitor, is contraindicated due to the potential for decreased cobicistat plasma concentrations and development of resistance. The coadministration of cobicistat plus elvitegravir with carBAMazepine caused an increase in carBAMazepine Cmax by 40% and AUC by 43% .

How To Manage Interaction
Concurrent use of carBAMazepine and cobicistat is contraindicated due to the potential for decreased cobicistat plasma concentrations and/or increased concentrations of carBAMazepine. Reduced virologic response and possible viral resistance and/or increased carBAMazepine side effects may occur.

A) In a drug interaction pharmacokinetic study, coadministration of carBAMazepine 200 mg twice daily with elvitegravir/cobicistat 150 mg/150 mg once daily resulted in a mean ration of carBAMazepine AUC and Cmax of 1.43 (90% CI, 1.36 to 1.52) and 1.4 (90% CI, 1.32 to 1.49) respectively and carBAMazepine-10,11 epoxide AUC and Cmax of 0.65 (90% CI, 0.63 to 0.66) and 0.73 (90% CI, 0.7 to 0.78) respectively .
Carbamazepine Overview

Carbamazepine is used alone or in combination with other medications to control certain types of seizures in people with epilepsy. It is also used to treat trigeminal neuralgia (a condition that causes facial nerve pain). Carbamazepine extended-release capsules (Equetro brand only) are also used to treat episodes of mania (frenzied, abnormally excited or irritated mood) or mixed episodes (symptoms of mania and depression that happen at the same time) in patients with bipolar I disorder (manic-depressive disorder; a disease that causes episodes of depression, episodes of mania, and other abnormal moods). Carbamazepine is in a class of medications called anticonvulsants. It works by reducing abnormal electrical activity in the brain.
Cobicistat Overview

Cobicistat is used to increase the amounts of atazanavir (Reyataz, in Evotaz ) in adults and children weighing at least 77 pounds (35 kg) or darunavir (Prezista, in Prezcobix ) in adults and children weighing at least 88 pounds (40 kg) in the blood when these medications are used to treat human immunodeficiency virus (HIV). Cobicistat is in a class of medications called pharmacokinetic boosters. It works by increasing the amount of atazanavir or darunavir in the body so that they can have a greater effect.
