Increased aprepitant exposure and an increased risk for aprepitant toxicity


Interaction Summary

Coadministration of aprepitant and fluconazole should be avoided. Coadministration with moderate CYP3A4 inhibitors has been associated with a 2-fold increase in aprepitant plasma concentrations. Concomitant use of a CYP3A4 substrate, such as fosaprepitant/aprepitant, with fluconazole (a moderate CYP3A4 inhibitor) may increase exposure to the CYP3A4 substrate and increase the risk for toxicity. Fluconazole-mediated enzyme inhibition may persist for 4 to 5 days after discontinuation. Use caution and carefully monitor if coadministered .

How To Manage Interaction

Coadministration of aprepitant and fluconazole should be avoided. Fluconazole Overview

  • Fluconazole is used to treat fungal infections, including yeast infections of the vagina, mouth, throat, esophagus (tube leading from the mouth to the stomach), abdomen (area between the chest and waist), lungs, blood, and other organs. Fluconazole is also used to treat meningitis (infection of the membranes covering the brain and spine) caused by fungus. Fluconazole is also used to prevent yeast infections in patients who are likely to become infected because they are being treated with chemotherapy or radiation therapy before a bone marrow transplant (replacement of unhealthy spongy tissue inside the bones with healthy tissue). Fluconazole is in a class of antifungals called triazoles. It works by slowing the growth of fungi that cause infection.

Aprepitant Overview

  • Aprepitant is used with other medications in adults and children 6 months of age and older to prevent nausea and vomiting that may occur after receiving cancer chemotherapy treatment. It is also used with other medications in adults and children 6 months of age and older to prevent delayed nausea and vomiting that may occur several days after receiving certain chemotherapy medications. Aprepitant is not used to treat nausea and vomiting that you already have. Aprepitant is in a class of medications called antiemetics. It works by blocking the action of neurokinin, a natural substance in the brain that causes nausea and vomiting.
