Interaction Summary
Eplerenone is primarily metabolized via the CYP3A isozyme. When eplerenone was coadministered with moderate CYP3A inhibitors (eg, erythromycin 500 mg twice daily, verapamil 240 mg once daily, saquinavir 1200 mg 3 times daily, or fluconazole 200 mg once daily), the eplerenone Cmax increased by 40% to 60% and the eplerenone AUC increased by 100% to 190%. Concomitant use of eplerenone and a moderate CYP3A inhibitor is not recommended due to the potential for significantly elevated eplerenone plasma concentrations. If concurrent use is unavoidable, do not exceed a dosage of 25 mg once daily in patients with congestive heart failure following a myocardial infarction. For patients with hypertension, initiate dosing with 25 mg once daily and do not exceed a dosage of 25 mg twice daily.

How To Manage Interaction

Fluconazole Overview

Fluconazole is used to treat fungal infections, including yeast infections of the vagina, mouth, throat, esophagus (tube leading from the mouth to the stomach), abdomen (area between the chest and waist), lungs, blood, and other organs. Fluconazole is also used to treat meningitis (infection of the membranes covering the brain and spine) caused by fungus. Fluconazole is also used to prevent yeast infections in patients who are likely to become infected because they are being treated with chemotherapy or radiation therapy before a bone marrow transplant (replacement of unhealthy spongy tissue inside the bones with healthy tissue). Fluconazole is in a class of antifungals called triazoles. It works by slowing the growth of fungi that cause infection.
Eplerenone Overview

Eplerenone is used alone or in combination with other medications to treat high blood pressure. Eplerenone is in a class of medications called mineralocorticoid receptor antagonists. It works by blocking the action of aldosterone, a natural substance in the body that raises blood pressure.
