Increased or decreased verapamil serum concentrations and potential toxicity or decreased efficacy


Interaction Summary

Coadministered tipranavir/ritonavir and verapamil may either increase or decrease serum concentrations of verapamil. Verapamil is a substrate of both CYP3A and the p-glycoprotein transporter system. The tipranavir/ritonavir combination inhibits CYP3A (increasing verapamil exposure) and induces the p-glycoprotein transporter system (decreasing verapamil exposure). The resulting effect on verapamil metabolism is unpredictable. Caution is warranted with concomitant use of these medications. Patients should be monitored for both verapamil toxicity and decreased verapamil efficacy.

How To Manage Interaction

Use caution with the coadministration of verapamil and tipranavir/ritonavir. It is difficult to predict the effect of concurrent use of tipranavir/ritonavir with verapamil due to the opposing impact of CYP3A inhibition and p-glycoprotein induction. Monitor patients for signs and symptoms of both calcium channel blocker toxicity and decreased efficacy. Decrease or increase verapamil dosage as appropriate.

Verapamil Overview

  • Verapamil is used to treat high blood pressure and to control angina (chest pain). The immediate-release tablets are also used alone or with other medications to prevent and treat irregular heartbeats. Verapamil is in a class of medications called calcium-channel blockers. It works by relaxing the blood vessels so the heart does not have to pump as hard. It also increases the supply of blood and oxygen to the heart and slows electrical activity in the heart to control the heart rate.

These changes include eating a diet that is low in fat and salt, maintaining a healthy weight, exercising at least 30 minutes most days, not smoking, and using alcohol in moderation.

Tipranavir Overview

  • Tipranavir is used with ritonavir (Norvir) and other medications to treat human immunodeficiency virus infection (HIV). Tipranavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although tipranavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
