Interaction Summary

Coadministration of aprepitant and warfarin resulted in a 14% decrease in INR 5 days after completion of aprepitant therapy. Monitor the patient's INR in the 2-week period, particularly at 7 to 10 days, following initiation of the 3-day regimen of aprepitant with each chemotherapy cycle, or following the administration of a single 40 mg dose for the prevention of postoperative nausea and vomiting .

How To Manage Interaction

Coadministration of fosaprepitant or aprepitant with warfarin may cause decreased plasma concentrations of warfarin which may result in a clinically significant decrease in International Normalized Ratio (INR) of prothrombin time. Monitor the patient's INR in the 2-week period, particularly at 7 to 10 days, following initiation of the 3-day regimen of aprepitant with each chemotherapy cycle, or following the administration of a single 40 mg dose for the prevention of postoperative nausea and vomiting .

A) When healthy subjects who were stabilized on chronic warfarin therapy were given a single 125-mg dose of oral aprepitant on day 1 and 80 mg/day on days 2 and 3, there was a 34% decrease in S(-) warfarin (a CYP2C9 substrate) trough concentration accompanied by a 14% decrease in the INR 5 days after complete dosing with aprepitant .

  • Warfarin is used to prevent blood clots from forming or growing larger in your blood and blood vessels. It is prescribed for people with certain types of irregular heartbeat, people with prosthetic (replacement or mechanical) heart valves, and people who have suffered a heart attack. Warfarin is also used to treat or prevent venous thrombosis (swelling and blood clot in a vein) and pulmonary embolism (a blood clot in the lung). Warfarin is in a class of medications called anticoagulants ('blood thinners'). It works by decreasing the clotting ability of the blood.

Aprepitant Overview

  • Aprepitant is used with other medications in adults and children 6 months of age and older to prevent nausea and vomiting that may occur after receiving cancer chemotherapy treatment. It is also used with other medications in adults and children 6 months of age and older to prevent delayed nausea and vomiting that may occur several days after receiving certain chemotherapy medications. Aprepitant is not used to treat nausea and vomiting that you already have. Aprepitant is in a class of medications called antiemetics. It works by blocking the action of neurokinin, a natural substance in the brain that causes nausea and vomiting.
