Levonorgestrel falls under Topical Drugs — the group Alchemilla interacts with.

While no side effects have been reported from orally using Alchemilla, a clinical study titled "Treatment of Aphthous Stomatitis with Topical Alchemilla vulgaris in Glycerine," published in Clinical Drug Investigation (https://pubmed.ncbi.nlm.nih.gov/17163290/), found that local mouth irritation occurred after applying a commercial product containing 3% alchemilla extract and glycerol (Aphtarine, Laboratoires Biosphere). Aphtarine was non-irritating in 68.2% of cases within 2-3 days, in 18.8% of cases within 4-7 days, and in 13.6% of cases with moderate irritation. Theoretically, combining Alchemilla with other potentially irritating topical products could increase the risk of local irritation and discomfort. The drug in this drug interaction monograph is available in a topical dosage form. There is no interaction expected from the oral dosage form of the drug.

Here's the good news: the concern on file is really about topical products that sit on the skin, and the way you'd use levonorgestrel for emergency contraception is by mouth (think Plan B or Levonelle). So when you swallow a levonorgestrel pill, there is no expected interaction with Alchemilla at all. The herb's tannins don't change how this hormone works or how well it prevents pregnancy.

The only flag worth knowing is that Alchemilla applied to skin or the mouth can sometimes cause mild local irritation, and pairing it with other irritating topical products could add to that. That's a skin-comfort issue, not something that affects your birth control. If you're ever unsure, your pharmacist is happy to double-check with you.

Alchemilla (lady's mantle) is a European herb traditionally used for digestive complaints, women's health issues, and as an astringent for skin and wounds. Solid human research is very limited, so its benefits remain largely unproven, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

What Is Levonorgestrel Used For?

Brand names: Plan B, Mirena, Levonelle One Step, Levonelle 1500, Norgeston, Skyla, Liletta, Kyleena

Levonorgestrel is used in two very different ways depending on the product. As an intrauterine system (IUS), Mirena and Liletta prevent pregnancy for up to 8 years and also treat heavy menstrual bleeding for up to 5 years in people who choose intrauterine contraception. Kyleena prevents pregnancy for up to 5 years, and Skyla prevents pregnancy for up to 3 years — neither Kyleena nor Skyla is indicated for heavy menstrual bleeding.

As an oral tablet, products such as AfterPill, AfterPlan, Amazon Basic Care Levonorgestrel, Econ Morning After, Emergency Contraceptive, and Empowered Choice are used as emergency (backup) contraception to reduce the chance of pregnancy after unprotected sex or contraceptive failure. The oral tablet is not intended for routine birth control.
