Moderate

HEPATOTOXIC DRUGS

Why this applies to Eltrombopag

Eltrombopag falls under HEPATOTOXIC DRUGS — the group Cassia Cinnamon interacts with.

Theoretically, large doses of cassia cinnamon might cause additive effects when used with hepatotoxic drugs.
There is some concern that ingesting large amounts of cassia cinnamon for an extended duration might cause hepatotoxicity in some people. Cassia cinnamon contains coumarin, which can cause hepatotoxicity in animal models. In humans, very high doses of coumarin from 50-7000 mg/day can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about pairing cassia cinnamon with eltrombopag (Promacta or Revolade): this isn't about cinnamon changing how the drug works or how much builds up. Eltrombopag already carries a known risk of stressing the liver, and your prescriber likely checks your liver enzymes while you're on it. Cassia cinnamon contains coumarin, which in large, regular doses can also be tough on the liver. So the worry is simply that two liver stressors stacked together could add up.

To be fair, this is a theoretical concern based on how these things behave, not something proven to happen. Everyday cooking amounts of cinnamon are very unlikely to matter. It's the concentrated, high-dose supplements taken for a long time that raise the flag. Please check with your pharmacist or doctor before starting one.

From our pharmacist’s overview on the Eltrombopag page

“Eltrombopag (Promacta, Revolade) boosts platelet production, and because the drug itself can stress the liver, this list warrants genuine care. Thirty-eight of the 55 supplements and herbs here can injure the liver on their own, so combining them stacks risk on an organ your doctor is already watching with blood tests.”

About Cassia Cinnamon

What is Cassia Cinnamon?

Cassia Cinnamon
  • Blood sugar support
  • Digestive upset
  • Antioxidant support
  • Flavoring and culinary use
  • Antimicrobial

Cassia cinnamon is the common, inexpensive cinnamon used in cooking, and it is also taken as a supplement, most often for blood sugar support. The evidence for its health benefits is mixed and far from proven, and because cassia is naturally high in coumarin, taking large amounts regularly may stress the liver. Use food amounts freely, but talk with a pharmacist or doctor before using high-dose supplements.

About Eltrombopag

What Is Eltrombopag Used For?

Brand names: Promacta, Revolade, Alvaiz

Eltrombopag treats thrombocytopenia (low platelet counts) in several different conditions. It is used for adults and children (age 1 and older, depending on the product) with persistent or chronic immune thrombocytopenia (ITP) — a condition where the immune system destroys platelets — when other treatments like steroids or immunoglobulins haven't worked well enough. It is also used to raise platelet counts in adults with chronic hepatitis C whose low platelet count prevents them from starting or staying on interferon-based antiviral therapy.

Eltrombopag tablets (including Promacta) are also approved — alone or combined with immunosuppressive therapy — for adults and certain children (age 2 and older) with severe aplastic anemia, a serious bone marrow failure condition. Alvaiz (a specific brand) shares most of these uses but has product-specific dosing that is not interchangeable milligram-for-milligram with other eltrombopag products. The oral suspension formulation is approved for ITP and for refractory severe aplastic anemia, but is not approved for first-line severe aplastic anemia in combination with immunosuppressive therapy.
