Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Gymnema Overview
Gymnema (Gymnema sylvestre) is a plant species native to India and Africa that has been used traditionally in Ayurvedic medicine for centuries to treat a range of conditions, including diabetes, obesity, and digestive problems.
Gymnema is thought to work by inhibiting the absorption of sugar in the intestines and by increasing the production of insulin in the pancreas. If applied topically to the tongue, it reduces the sweetness and bitter sensation of foods, which may deter excess sugar intake (the plant is sometimes referred to informally as 'the sugar destroyer').
One of the most popular natural medicines utilized for dietary supplements, an extract of the leaf (Gymnema leaf extract) is the most often standardized for use, although the root can be used as well.
Tofacitinib Overview

Tofacitinib is used alone or with other medications to treat rheumatoid arthritis (condition in which the body attacks its own joints causing pain, swelling, and loss of function) in adults who are unable to take or did not respond to one or more tumor necrosis factor (TNF) inhibitor medication(s). It is also used to treat psoriatic arthritis (a condition that causes joint pain and swelling and scales on the skin) in adults who are unable to take or did not respond to or tolerate one or more TNF inhibitor medication(s). Tofacitinib is used to treat ulcerative colitis (a condition which causes swelling and sores in the lining of the colon [large intestine] and rectum) in adults who are unable to take or who did not respond to one or more TNF inhibitor medication(s). It is also used to treat polyarticular juvenile idiopathic arthritis (PJIA; a type of childhood arthritis that affects five or more joints during the first six months of the condition, causing pain, swelling, and loss of function) in children 2 years and older who are unable to take or who did not respond to one or more TNF inhibitor medication(s). Tofacitinib is in a class of medications called Janus kinase (JAK) inhibitors. It works by decreasing the activity of the immune system.
