The most common side effects reported orally are abdominal discomfort, bloating, diarrhea, nausea, and vomiting. Rare serious effects include shiitake dermatitis (a skin reaction, sometimes with whole-body symptoms) from raw mushrooms, and in people who handle shiitake extensively, allergic reactions like asthma, rhinitis, and in severe cases, hypersensitivity pneumonitis from spore inhalation.

Mushroom mycelia extract
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Immunosuppressants
Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
In vitro evidence suggests that shiitake mushroom extracts stimulate immune function.
