Carotomax contains six active ingredients. Lycopene, zeaxanthin, astaxanthin, and lutein are carotenoids—pigments from plants that act as antioxidants in your body.
Vitamin A is a fat-soluble vitamin essential for vision and immune function. Alpha carotene is another plant pigment.
The capsules also contain soybean oil, beeswax, palm oil, soy lecithin, and gelatin as inactive ingredients.
Does it work?
Moderate evidence
The evidence for what this product can do varies. Lycopene shows possibly effective results for prostate cancer but possibly ineffective for bladder cancer and diabetes.
Vitamin A is effective for vitamin A deficiency and possibly effective for measles, aging skin, and a few other conditions. Lutein is possibly effective for age-related macular degeneration and cataracts.
Zeaxanthin is possibly effective for age-related macular degeneration. For astaxanthin and alpha carotene—or for most of the other conditions listed on the label—the evidence in our data is either insufficient to rate or not established.
How safe is it?
Well-documented data
Lycopene and lutein are generally well tolerated at typical supplement doses, though lycopene can cause stomach upset, diarrhea, or nausea in some people. Vitamin A is safe at recommended amounts but can be toxic at high doses because it builds up in your body over time—large amounts may cause liver damage, headaches, or skin problems.
Astaxanthin is generally well tolerated but may cause abdominal pain, diarrhea, or a red tint to stools; long-term safety data are limited. For pregnancy and breastfeeding, vitamin A at high doses poses real risks, so you'll want to talk with your doctor or pharmacist about whether this product is right for you if you're pregnant or nursing.
The safety of astaxanthin in pregnancy and breastfeeding is not established, so it's best to avoid it during these times.

Before taking Carotomax, double-check these medication types: retinoid medications (isotretinoin, tretinoin) because of major interaction risk with vitamin A; blood thinners and antiplatelet drugs (warfarin, aspirin, clopidogrel) because of moderate bleeding risk from lycopene and vitamin A; tetracycline antibiotics because of moderate risk of increased pressure in the skull; diabetes medications because of moderate risk of low blood sugar from zeaxanthin; and any drugs metabolized by liver enzymes CYP3A4 or CYP2B6 because astaxanthin may reduce their effectiveness.

The most serious interaction involves vitamin A, one of the product's ingredients, which can dangerously amplify the effects of retinoid medications—acne treatments like isotretinoin (Accutane) and tretinoin (Retin-A) fall into this category. Taking vitamin A supplements alongside retinoids could push vitamin A levels to toxic levels.

Vitamin A also interacts with blood thinners like warfarin (Coumadin), liver-damaging drugs, and tetracycline antibiotics (used for acne and infections). At high doses, vitamin A can thin the blood further, stress the liver, or combine with tetracyclines to raise pressure inside the skull.
Lycopene, another ingredient here, may increase bleeding risk when combined with blood thinners or antiplatelet drugs like aspirin and clopidogrel (Plavix).
Zeaxanthin could theoretically lower blood sugar too much if you're on diabetes medications. Astaxanthin may reduce how your body processes certain medications broken down by the liver—specifically drugs metabolized through CYP3A4 and CYP2B6 pathways.

Astaxanthin
Cytochrome P450 2B6 (Cyp2B6) Substrates
Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP2B6.
In vitro research shows that astaxanthin induces cytochrome CYP2B6 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Vitamin A
Retinoids
Concomitant use of retinoids with vitamin A supplements might produce supratherapeutic vitamin A levels.
Retinoids, which are vitamin A derivatives, could have additive toxic effects when taken with vitamin A supplements.
Hepatotoxic Drugs
Theoretically, taking high doses of vitamin A in combination with other potentially hepatotoxic drugs might increase the risk of liver disease.
The tolerable upper intake level (UL) is the highest level of intake that is likely to pose no risk of adverse effects. Doses of vitamin A above the UL can cause hepatotoxicity, ranging from elevated liver enzymes to liver failure.
Tetracycline Antibiotics
Theoretically, taking tetracycline antibiotics with high doses of vitamin A can increase the risk of pseudotumor cerebri.
Benign intracranial hypertension (pseudotumor cerebri) can occur with tetracyclines and with acute or chronic vitamin A toxicity. Case reports suggest that taking tetracyclines and vitamin A concurrently can increase the risk of this condition. Avoid high doses of vitamin A in people taking tetracyclines chronically.
Warfarin (Coumadin)
Theoretically, high doses of vitamin A could increase the risk of bleeding with warfarin.
Vitamin A toxicity is associated with hemorrhage and hypoprothrombinemia, possibly due to vitamin K antagonism. Advise patients taking warfarin to avoid doses of vitamin A above the tolerable upper intake level of 10,000 IU/day for adults.
Lycopene
Anticoagulant/Antiplatelet Drugs
Theoretically, taking lycopene with anticoagulant or antiplatelet drugs might increase the risk of bleeding.
In vitro research shows that lycopene has antiplatelet effects.
Zeaxanthin
Antidiabetes Drugs
Theoretically, taking zeaxanthin with antidiabetes drugs might increase the risk of hypoglycemia.
In an animal diabetic model, zeaxanthin has hypoglycemic effects. However, population research has found that increasing intake of dietary zeaxanthin plus lutein does not decrease the risk of developing type 2 diabetes.
