The capsule itself is made from gelatin and rice flour, which are inactive ingredients.

Not established
Cascara sagrada is rated possibly ineffective for bowel preparation — the evidence doesn't support using it for that purpose. For constipation, the evidence we hold is insufficient to rate whether it works or doesn't.

The biggest concern is with blood thinners like warfarin (Coumadin). Cascara sagrada's laxative effect can cause diarrhea, which can make warfarin work harder and raise your bleeding risk.
Similarly, the stimulant laxative action can lower your potassium levels, which becomes a real problem if you're also taking corticosteroids, water pills (diuretics), or digoxin (Lanoxin, a heart medication) — all of which can cause potassium loss on their own. When these add up together, low potassium can trigger serious heart rhythm problems, especially with digoxin.
You should also avoid taking cascara with other stimulant laxatives, since they'd compound the fluid loss and electrolyte imbalance. Finally, cascara may reduce the effectiveness of certain medications broken down by your liver's CYP3A4 enzyme system — a Minor interaction, but still worth checking if you're on drugs like some cholesterol medications or immunosuppressants.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.
