Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: cilantro leaf extract (in dry form). The liquid also contains alcohol, water, and glycerin as inactive ingredients to deliver the extract.
Does it work?
Couldn't assess
The evidence for cilantro isn't established in the data we hold. The monograph shows insufficient reliable evidence to rate it for anorexia, anxiety, constipation, diabetes, diarrhea, or indigestion.
If you're considering it for a specific condition, talk with your doctor or pharmacist about what the current evidence actually supports.
How safe is it?
Well-documented data
Coriander is generally well tolerated when used in food amounts. However, concentrated medicinal doses — like those in a liquid extract — lack solid safety data, so use caution and stick with your doctor's or pharmacist's guidance on dosing.
Side effects are rare but worth knowing about. Topically, it can cause contact dermatitis, itching, or a burning sensation in sensitive people.
Taken by mouth, it's usually well tolerated, but anaphylaxis (a severe allergic reaction) has been reported in rare cases, particularly in people sensitized through occupational exposure. If you have a known spice allergy or have had allergic symptoms to coriander before, be especially cautious.
Meds to double-check
Moderate interaction found
Before taking this product, double-check with your doctor or pharmacist if you take any blood sugar medications (antidiabetes drugs), blood pressure medications (antihypertensive drugs), sedating drugs (CNS depressants like sleep aids or anxiety medications), or medications that increase sun sensitivity (photosensitizing drugs). All of these have Moderate interactions with cilantro extract.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Cilantro extract may be an option if you're interested in herbal support, but the evidence for what it actually does isn't strong. If you take blood sugar, blood pressure, or sedating medications, or anything that makes you sun-sensitive, you'll need to check with your doctor or pharmacist before starting — the interactions are real.
Anyone with a spice allergy should be careful as well.

Cilantro by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Hawaii Pharm's Cilantro Liquid contains cilantro leaf extract, which is derived from coriander and has documented interactions with several medication types.
The most serious concern is with blood sugar medications (antidiabetes drugs) — coriander can lower blood glucose levels, so taking it alongside insulin or oral diabetes drugs may increase the risk of low blood sugar (hypoglycemia). You'll need to monitor your blood sugar closely, and your doctor may need to adjust your medication dose.
Read the full breakdown — every affected drug type, severity by severity
Coriander also interacts with blood pressure medications (antihypertensive drugs) and sedating drugs (CNS depressants), both at Moderate severity. It may lower blood pressure further or add to drowsiness.
Additionally, if you take a medication that makes your skin sun-sensitive (photosensitizing drugs), coriander may increase that risk.
Altogether, these interactions span 717 individual medications. Please use the medication checker below to search your exact prescriptions before you start this product, and talk it over with your doctor or pharmacist.
