Taken by mouth, Guduchi is generally well tolerated, though headache and nasal pain were reported in clinical trials. However, liver injury is a serious concern: case reports document 49 patients developing autoimmune hepatitis, acute hepatitis, worsening liver disease, or acute liver failure after taking Tinospora cordifolia alone or in combination supplements for a median of 42 to 90 days.

Of those, two needed liver transplants and four died. Because of this risk, use it only under guidance and be alert to signs of liver trouble.

The safety data advises against use in pregnancy. There isn't enough reliable information to know whether it's safe while breastfeeding — talk with your doctor or pharmacist for personalized advice if you're nursing or planning to.

Antidiabetes Drugs

Theoretically, Tinospora cordifolia might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Clinical research in adults with type 2 diabetes shows that Tinospora cordifolia can reduce fasting blood glucose and glycated hemoglobin. Additionally, animal research shows that Tinospora cordifolia has hypoglycemic effects.
