Synastat contains one active ingredient: red yeast rice. This is a fermented rice product that naturally contains lovastatin, a statin-class compound that helps lower cholesterol much like prescription statins do.
The product also contains inactive ingredients — brown rice, soy, and hypromellose (a capsule material) — to fill out and form the capsule.
Does it work?

Red yeast rice in Synastat is rated possibly effective for two uses: helping manage abnormal cholesterol and triglycerides in people with HIV/AIDS and possibly lowering the risk of heart attack. The evidence supporting these uses is not conclusive, so results vary person to person.
How safe is it?
Well-documented data
Red yeast rice is generally well tolerated when taken by mouth, but side effects do occur. Common ones include abdominal discomfort, diarrhea, dizziness, flatulence, headache, heartburn, muscle pain, and nausea.
Rare but serious effects include liver damage and rhabdomyolysis (severe muscle breakdown), both linked to the lovastatin it contains. Because product quality varies widely, you may also get unintended contaminants that could harm your kidneys.
During pregnancy, red yeast rice is likely unsafe — it works like a statin drug, and statins are not safe in pregnancy. There's not enough safety data to recommend it while breastfeeding, so talk with your doctor or pharmacist before using it if you're nursing.

Directions for Use: Take 2 capsules 2 times per day (morning and evening) with water at mealtimes.
Precautions
Do not take more than 4 capsules in a 24-hour period. Immediately discontinue use if you experience any unexplained muscle pain or tenderness, especially if accompanied by flu-like symptoms.
Keep out of reach of children.
As with any dietary supplement, consult a physician if you are taking any prescription medication. Synastat by Sylvan Therapeutics contains red yeast rice, which acts like a statin drug in your body and carries real interaction risks.
The most serious concern is with cyclosporine (Neoral, Sandimmune), an immunosuppressant — combining them could raise lovastatin levels dramatically (5- to 20-fold) and cause muscle damage (myopathy) or a severe breakdown of muscle tissue (rhabdomyolysis).

Red yeast rice also interacts with several other drug types at moderate severity. It shouldn't be mixed with gemfibrozil (Lopid, a triglyceride-lowering drug) or high-dose niacin because of rhabdomyolysis risk.
CYP3A4 inhibitors — drugs your liver uses to break down lovastatin — can raise lovastatin levels unexpectedly. Other statins combined with red yeast rice may increase statin side effects.
Drugs that damage the liver (hepatotoxic drugs) carry an added risk when paired with this supplement.
Altogether, these interactions span 459 individual medications. Because red yeast rice's lovastatin content varies unpredictably between batches, the actual risk is hard to predict.

Red Yeast Rice
Cyclosporine (Neoral, Sandimmune)
Theoretically, taking red yeast rice in combination with cyclosporine might increase the risk of myopathy.
Red yeast rice contains varying levels of the statin drug lovastatin. Cyclosporine has been reported to increase plasma levels of lovastatin by 5- to 20-fold, resulting in reports of myopathy and rhabdomyolysis.
Cytochrome P450 3A4 (Cyp3A4) Inhibitors
Theoretically, drugs that inhibit the CYP3A4 enzymes might increase levels of lovastatin from red yeast rice.
Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.
Gemfibrozil (Lopid)
Theoretically, taking red yeast rice in combination with gemfibrozil might increase the risk of rhabdomyolysis.

Hepatotoxic Drugs
Theoretically, concomitant use might increase the risk of liver damage.
Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.
Hmg-Coa Reductase Inhibitors ("Statins")
Theoretically, taking red yeast rice with other statins might increase the risk of potential adverse effects.
Red yeast rice contains varying levels of the statin drug lovastatin and might result in supratherapeutic levels when used with other statins. Based on evaluation of data from the US Food and Drug Administration's adverse event reporting system (FAERS), it is recommended that red yeast rice products be avoided in people taking prescription statins.
Niacin
Theoretically, taking red yeast rice in combination with high-dose niacin might increase the risk of rhabdomyolysis.
