The evidence for tart cherry is mixed and often thin. It's possibly effective for athletic performance — the strongest claim in our data.
For everything else on file — ADHD, brain function, muscle soreness after exercise, respiratory infections after intense exercise, and fibromyalgia — the evidence is insufficient to rate. That doesn't mean it doesn't work; it means we don't have enough reliable research yet to say either way.
How safe is it?

Tart cherry is generally well tolerated when you eat the whole fruit. The catch: concentrated supplements haven't been studied as thoroughly as whole food, so caution is warranted.
The most common side effects from the concentrate are abdominal pain, loose stools, and general stomach upset — a clinical trial found that about 29% of children taking tart cherry juice concentrate reported belly pain. A rare skin reaction possibly linked to cherry allergy has been reported.
