Lithium reduces inositol levels in the brain by inhibiting the inositol monophosphatase enzyme, which is involved in synthesis of inositol. Levels of inositol in the systemic circulation are determined primarily by dietary intake, and do not seem to be significantly affected by lithium. The effect of lithium on brain inositol levels might contribute to its therapeutic effect in bipolar disorder. Animal research suggests that inositol supplementation may reduce the therapeutic effects of lithium, whereas dietary inositol restriction may augment the effects of lithium. It has also been suggested that reduction of tissue inositol levels might contribute to adverse effects of lithium; a small study reported benefit with inositol supplements in lithium-induced psoriasis.
Inositol is a naturally occurring compound that plays a role in cell signaling throughout the body, including in the brain. Lithium appears to block an enzyme the brain uses to recycle inositol, which can modestly lower inositol levels in brain tissue. Interestingly, some researchers think this effect may actually be part of how lithium helps with bipolar disorder. The good news is that inositol levels in the bloodstream are mainly controlled by what you eat and do not seem to be meaningfully affected, so for most people taking lithium, this is not something that requires attention. If you have questions or notice any unusual symptoms, it is always fine to bring them up with your pharmacist or doctor.
