I want to start taking a supplement that contains, among other things, pomegranate powder. I am currently taking atorvastatin and have heard that pomegranate may cause problems when taken with atorvastatin. Is there any truth to that?

There is a potential interaction between pomegranate and Lipitor (atorvastatin), but it is thought to be a relatively minor one and one that is unlikely to have a significant clinical impact.

Interaction Summary - Lipitor With Pomegranate

In theory, pomegranate could interact with drugs that are metabolized by an enzyme called 'CYP3A4', a very prominent metabolizing enzyme in the body that is responsible for the breakdown of a large number of drugs, including Lipitor (atorvastatin).

Inhibition of this enzyme could slow the metabolism of Lipitor, resulting in increased concentrations in the blood, potentially increasing the risk of side effects occurring.

In fact, the prescribing information for Lipitor specifically warns about taking the drug with CYP3A4 inhibitors:

LIPITOR is a substrate of CYP3A4 and transporters (e.g., OATP1B1/1B3, P-gp, or BCRP). LIPITOR plasma levels can be significantly increased with concomitant administration of inhibitors of CYP3A4 and transporters.

The good news, as it relates to pomegranate, is that most studies suggest an interaction with CYP3A4 drugs is unlikely to be a big concern in real-world scenarios.

Some lab studies have reported that pomegranate juice does seem to slow down CYP3A4 activity, which is why an interaction comes up at all in our interaction checker, but tests in humans have generally shown no significant impact on drug levels.

There was a study published in 2015 that evaluated whether or not pomegranate juice affected simvastatin concentrations. Simvastatin is another 'statin' drug very similar to Lipitor and is also metabolized by CYP3A4. The study, published in the European Journal of Drug Metabolism and Pharmacokinetics, concluded the following:

These results suggest that pomegranate juice affects little on the disposition of simvastatin in humans. Pomegranate juice does not seem to have a clinically relevant inhibitory potential on CYP3A4 activity.

Overall, it's unlikely that pomegranate will significantly impact Lipitor concentrations or increase the risk of side effects.

As I wrote above, some small lab studies have indicated pomegranate seems to have a minor effect on CYP3A4 levels; human studies haven't shown any clinical effects.
