Jul 15, 2022- Insulin is prescribed and dosed in units based on its glucose-lowering effect, not in milligrams, and you should always follow your prescriber's instructions and your specific insulin product's guidance. Converting insulin units to milliliters depends on your insulin's concentration (such as U-100 or U-500), so the same number of units can mean very different liquid volumes. Do not attempt to convert insulin doses to milligrams for injection or dosing decisions, as different insulin products have different milligram equivalents.
It is very uncommon to see insulin doses written in milligrams (mg). Insulin products are labeled and prescribed in units, usually as a concentration such as U-100, which means 100 units of insulin per 1 mL of liquid.
Bottom line: For real-world dosing, insulin should be understood in units, not milligrams. The most useful conversion for patients and caregivers is usually units to milliliters, not units to milligrams.
Safety note: Do not use a milligram calculation to draw up, inject, or switch insulin products. Always follow the dose written by your prescriber and the instructions for your specific insulin pen, vial, syringe, or pump.
Why Insulin Is Measured In Units Instead Of Milligrams
A unit of insulin is a measure of biologic activity, meaning the glucose-lowering effect of the insulin. This is different from a metric weight, such as milligrams.
This matters because insulin products are not all identical by weight, formulation, absorption, onset, peak, or duration. Insulin units are used because they are the clinically meaningful dosing measure.
In practice, a prescription saying “inject 10 units” is also much safer and easier to understand than trying to dose insulin by milligrams.
Can You Convert Insulin Units To Milligrams?
For routine use, you generally should not convert insulin units to milligrams.
There is a reference conversion for pure human insulin: 1 International Unit (IU) of human insulin activity corresponds to 0.0347 mg of human insulin. However, this is a reference-standard value and should not be treated as a universal conversion for every insulin product.
That distinction is important. For example, FDA notes that 1 IU corresponds to the activity contained in 0.0347 mg of human insulin or 0.0363 mg of insulin glargine. This means the old shortcut of saying “1 unit of any insulin equals 0.0347 mg” is not accurate.
Because there is no single safe milligram conversion that applies to all insulin products, this article does not provide a general insulin unit-to-mg calculator.
Converting Insulin Units To Milliliters
The more practical conversion is between units and milliliters (mL). This depends on the concentration of the insulin product.
Insulin concentrations are written as “U” values:
- U-40 = 40 units per 1 mL
- U-100 = 100 units per 1 mL
- U-200 = 200 units per 1 mL
- U-300 = 300 units per 1 mL
- U-500 = 500 units per 1 mL
The formula is:
Milliliters = Units ÷ Concentration
For example:
- 50 units of U-100 insulin = 50 ÷ 100 = 0.5 mL
- 50 units of U-500 insulin = 50 ÷ 500 = 0.1 mL
This is why the concentration matters so much. The same number of insulin units can be a very different liquid volume depending on whether the product is U-100, U-200, U-300, or U-500.

Important Syringe And Pen Safety
If you use an insulin pen, you usually dial the prescribed number of units. You should not try to convert the pen dose into milliliters unless your healthcare provider specifically instructs you to do so.
If you use a vial and syringe, it is very important to use the correct syringe for the insulin product. This is especially important with concentrated insulin products such as U-500 insulin.
For example, U-500 insulin contains 5 times as much insulin per mL as U-100 insulin. Using the wrong syringe or trying to do your own conversion can cause a serious overdose or underdose.
With Humulin R U-500 specifically, the prescribing information says to use only a U-500 insulin syringe with the vial and not to transfer insulin from the U-500 pen into a syringe.
Call your pharmacist or prescriber before injecting if your prescription, syringe, pen, or dose instructions do not match or if you are unsure whether your insulin is U-100, U-200, U-300, or U-500.
Human Insulin Vs. Insulin Analogs
Another reason insulin is not usually discussed in milligrams is that there are different types of insulin products.
Human Insulin
Human insulin products include:
- Regular human insulin
- NPH insulin, also called isophane insulin
Regular human insulin is generally considered short-acting. NPH insulin is intermediate-acting and is formulated to slow absorption and extend its duration of action.
Insulin Analogs
Insulin analogs are modified forms of insulin. They are designed to change how quickly the insulin starts working, when it peaks, or how long it lasts.
Examples of rapid-acting insulin analogs include:
- Insulin aspart
- Insulin lispro
- Insulin glulisine, sold under the brand name Apidra
Examples of long-acting insulin analogs include:
- Insulin glargine
- Insulin degludec
- Insulin detemir
These products are still dosed in units, but that does not mean each unit has the exact same milligram weight. Some analogs may be described as having comparable glucose-lowering effect on a unit basis, but they can still differ in onset, peak, duration, concentration, formulation, and mass per unit.
Switching Between Insulin Products
Some insulin switches may start on a unit-to-unit basis, but this depends on the specific products involved and the patient’s situation. Insulin products should not be switched based only on a milligram calculation.
Your prescriber may need to adjust the dose based on:
- The type of insulin being started or stopped
- The insulin concentration
- Blood glucose readings
- Risk of hypoglycemia
- Kidney or liver function
- Diet, activity level, illness, or other medications
Do not change insulin products, concentrations, or doses unless your healthcare provider tells you to do so.
