In terms of what to do if you miss a dose of your Medrol Dose Pack, the recommendation is:

  • Take the missed dose as soon as you remember, but, if it's almost time for your next dose, simply skip the missed dose and wait for the next one instead.
  • Do not take 2 doses at the same time or extra doses to 'catch up'.

Detailed Answer

A Medrol Dose Pack contains methylprednisolone, a glucocorticoid. It is also sometimes known simply as a 'steroid'.

Methylprednisolone is used for a wide variety of purposes, but the Dose Pack specifically is most commonly used for acute inflammatory conditions, such as bronchitis and musculoskeletal injuries. It is also commonly used to help individuals recover after an asthma attack.

A Medrol Dose Pack consists of twenty-one, 4mg tablets, taken over the course of 6 days.

You start at a higher dose on day one (taking 24mg total) and taper down over the course of 6 days. The recommended way to take the pack is as follows:

  • Day 1: Take 2 tablets (8 mg) before breakfast, 1 tablet (4 mg) after lunch, 1 tablet (4 mg) after supper, and 2 tablets (8 mg) at bedtime.
  • Day 2: Take 1 tablet (4 mg) before breakfast, 1 tablet (4 mg) after lunch, 1 tablet (4 mg) after supper, and 2 tablets (8 mg) at bedtime.
  • Day 3: Take 1 tablet (4 mg) before breakfast, 1 tablet (4 mg) after lunch, 1 tablet (4 mg) after supper, and 1 tablet (4 mg) at bedtime.
  • Day 4: Take 1 tablet (4 mg) before breakfast, 1 tablet (4 mg) after lunch, and 1 tablet (4 mg) at bedtime.

Why Does The Medrol Dose Pack Start At A High Dose And Decrease Every Day?

High doses of steroids, like methylprednisolone, taken for long periods of time, can cause a side effect known as HPA axis suppression.

The HPA (hypothalamic-pituitary-adrenal) axis is a complex system that plays a critical role in regulating the body's stress response, among other things.

Steroids work by mimicking the effects of the body's natural stress hormones, which are produced by the adrenal glands.

When steroids are taken for a long time, this causes a feedback loop in the HPA axis, which signals the hypothalamus and pituitary gland to stop producing and releasing the hormones that normally stimulate the adrenal glands. In other words, with prolonged use of steroids, the HPA axis can become suppressed.

The Medrol Dose Pack starts at a higher dosage on day one (6 tablets of methylprednisolone, equivalent to 24mg) and decreases by one tablet every day until the pack is gone. This is known as a tapering dosage regimen. 

This tapering dosage is designed to allow the body to adjust to the changes in hormone levels that taking steroids can cause, gradually and safely, which reduces the likelihood of experiencing withdrawal symptoms or a sudden change in symptoms from the condition you are treating.

It also helps to minimize the potential side effects of steroids, such as weight gain, mood changes, insomnia, anxiety, and elevated blood sugar levels.

Now, there are a number of studies that suggest that tapering steroids isn't actually necessary for those who are only taking short courses (less than 2 weeks), and the Medrol Dose Pack would certainly fit in this category.

However, everyone's medical situation is different, and there are a number of factors that can influence whether or not tapering a steroid regimen would be recommended for any one individual (e.g., dose, duration of use, age, health status, etc...).

Tapering a steroid, like Medrol, even after taking it for a short period of time, is perfectly safe to do, and certainly can help to avoid certain side effects associated with steroid withdrawal.

What To Do If You Miss A Dose Of A Medrol Dose Pack?

As mentioned in the previous section, for short-term use of steroids, such as a Medrol dose pack, the duration of treatment is typically too short to cause significant changes in hormone levels that would require a taper. Most clinical studies report that it takes at least 2 weeks of continuous dosing of steroids to start to see issues occurring. This isn't applicable to everyone, but it is for most people.

Along the same lines, missing a dose during the taper (one tablet in your dose) is unlikely to cause any significant problems.

Nevertheless, it is important to follow the recommendations the drug packaging provides regarding a missed dose:

  • Take the missed dose as soon as you remember but if it is almost time for the next dose, just skip it and take the next dose at your normally scheduled time.
  • Do not double up or take extra doses in an attempt to catch up.

All of this being said, missing some doses or not tapering at all certainly could affect some individuals, and it's not uncommon that some side effects from Medrol, like anxiety and insomnia, can be worse if a taper is not undertaken or not done correctly.
