Coadministration of aripiprazole (CYP3A substrate) and vitamin D (CYP3A inducer) may decrease the serum concentration of aripiprazole and, therefore, decrease its efficacy. Higher vitamin D levels were associated with a greater likelihood of insufficient blood concentrations of antipsychotic drugs in a small study. In patients with schizophrenia where vitamin D supplementation is deemed clinically indicated, consider aripiprazole therapeutic drug monitoring-guided dosage adjustment to prevent worsening symptoms1.

a) In a small study with 12 serum concentrations of aripiprazole, dose-adjusted serum concentrations of aripiprazole were significantly decreased with vitamin D in patients with schizophrenia (N=121; 107 antipsychotic serum concentrations evaluable; 80 serum vitamin D levels evaluable). Metabolite to parent compound ratio (MPR), a direct measure of metabolizing enzyme activity, showed a non-significant positive correlation. Among serum concentrations of quetiapine and the active moiety of aripiprazole (n=23), 11 concentrations were in patients with vitamin D levels below the median and 12 with vitamin D levels above the median; of those with concentrations below the median, 91% had drug concentrations above the lower limit of the therapeutic reference range compared with 50% among patients with vitamin D levels above the median. In summary, patients with vitamin D levels above the median had a significantly higher proportion of drug concentrations below the lower limit of the therapeutic range 1.
