What happens

Increased colchicine exposure and an increased risk of colchicine toxicity

Interaction Deep Dive

Colchicine is a CYP3A4 and P-gp substrate. Toxicities have been reported when colchicine was coadministered with clarithromycin, a dual CYP3A4 and P-gp inhibitor. Toxicities have also occurred with colchicine administered with a CYP3A4 inhibitor and colchicine administered with a P-gp inhibitor3. Concomitant use of colchicine with P-gp inhibitors is contraindicated because life-threatening and fatal colchicine toxicity has been reported in these patients with colchicine taken at therapeutic doses 1. Also such use is contraindicated in patients with renal or hepatic impairment, as life-threatening and fatal drug interactions have occurred. In patients with normal renal and hepatic function 2, if coadministration of colchicine and a P-gp inhibitor is required, adjust the dose of colchicine by either reducing the daily dose or reducing the dose frequency. Monitor closely for colchicine toxicity 3.
