Increased cabazitaxel exposure

Avoid coadministration of cabazitaxel (a CYP3A4 substrate) with strong CYP3A4 inhibitors, as it may cause increased cabazitaxel exposure1. In a study, cabazitaxel exposure increased when patients were coadministered ketoconazole (strong CYP3A4 inhibitor) 2. If concomitant use is required, consider a 25% cabazitaxel dose reduction 1.

a) Concomitant use of cabazitaxel 5 mg/m(2) plus CISplatin 75 mg/m(2) IV for 75 cycles with ketoconazole 400 mg orally once daily for a median of 10 days (range, 1 to 17 days) resulted in a 20% decrease in cabazitaxel clearance (geometric mean ratio [GMR], 0.8 [90% CI, 0.55 to 1.15]) and a 25 % increase in AUC (GMR, 1.25 [90% CI, 0.86 to 1.81]) in a study in patients with advanced solid tumors (n=20; 24% lung as primary tumor site). Cabazitaxel Cmax decreased from 27.3 to 19.1 nanograms(ng)/mL, t(1/2) was unchanged, and Vd at steady-state increased from 2450 to 2970 L. CISplatin did not contribute to this affect since it is not a CYP3A inhibitor 2.
