Potentiation of hypotensive effects


Interaction Summary

Symptomatic postural hypotension has occurred in patients receiving simultaneous doses of doxazosin 4 milligrams (mg) and sildenafil 25 mg. Mean reductions of supine systolic and diastolic blood pressure have been observed in patients treated concurrently with doxazosin and sildenafil. Use the lowest dose of sildenafil in patients already maintained on an alpha-1 adrenergic blocker, and initiate therapy with alpha-1 adrenergic blockers at the lowest dose in patients receiving an optimized dose of sildenafil .

How To Manage Interaction

If co-administration is required, the patient should be stable on the alpha-1 adrenergic blocker prior to the initiation of sildenafil, and the lowest dose of sildenafil should be used. Conversely, in those patients already taking an effective dose of sildenafil, alpha-1 blocker therapy should be initiated at the lowest dose.

Sildenafil Overview

  • Sildenafil (Viagra) is used to treat erectile dysfunction (impotence; inability to get or keep an erection) in men. Sildenafil (Revatio) is used to improve the ability to exercise in adults and children 1 year of age and older with pulmonary arterial hypertension (PAH; high blood pressure in the vessels carrying blood to the lungs, causing shortness of breath, dizziness, and tiredness). Sildenafil is in a class of medications called phosphodiesterase (PDE) inhibitors. Sildenafil treats erectile dysfunction by increasing blood flow to the penis during sexual stimulation. This increased blood flow can cause an erection. Sildenafil treats PAH by relaxing the blood vessels in the lungs to allow blood to flow easily.

  • If you are taking sildenafil to treat erectile dysfunction, you should know that it does not cure erectile dysfunction or increase sexual desire. Sildenafil does not prevent pregnancy or the spread of sexually transmitted diseases such as human immunodeficiency virus (HIV).

Alfuzosin Overview

  • Alfuzosin is used in men to treat symptoms of an enlarged prostate (benign prostatic hyperplasia or BPH), which include difficulty urinating (hesitation, dribbling, weak stream, and incomplete bladder emptying), painful urination, and urinary frequency and urgency. Alfuzosin is in a class of medications called alpha blockers. It works by relaxing the muscles in the prostate and bladder to allow urine to flow more easily.
