Here's the gist for Tibolone (Livial), which you likely take to ease menopause symptoms. Tibolone is a bit unique: once in your body it breaks down into different forms, some that act like estrogen, some like progesterone, and some that act like testosterone (androgens). Androstenetrione blocks aromatase, the enzyme that turns testosterone-like hormones into estrogen-like ones. So in theory, taking it alongside tibolone could tilt the balance toward more androgen activity and less estrogen.
What might that look like? Possibly more androgen-type effects such as acne, oily skin, or unwanted hair growth, and maybe weaker relief of hot flashes. That said, this is a minor, theory-based concern with weak evidence, not something proven in people.
