Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the good news about Cabtreo (clindamycin, adapalene, and benzoyl peroxide): it's a topical gel you smooth onto your skin, not something you swallow. The interaction on file describes kaolin binding to oral clindamycin in the gut and slowing how fast it's absorbed. Since you're applying this product to your face, kaolin taken by mouth for diarrhea really shouldn't cross paths with it in any meaningful way.

So in practical terms, using a kaolin antidiarrheal while treating your acne with Cabtreo isn't a combination I'd lose sleep over. If you're ever prescribed clindamycin as a pill or capsule down the road, that's a different story, so keep it in mind. As always, run your specific routine by your pharmacist if you have any doubts.

From our pharmacist’s overview on the Clindamycin, Adapalene, Benzoyl Peroxide page

“Someone using Cabtreo (clindamycin, adapalene, and benzoyl peroxide) for acne is usually more worried about dryness and peeling than about supplement clashes, and the list here backs that up: only four ingredients on file, three moderate and one minor, nothing major. The main theme is absorption through the skin.”

About Kaolin

What is Kaolin?

Kaolin
  • Diarrhea
  • Upset stomach and nausea
  • Skincare and facial masks
  • Soothing minor skin irritation
  • Excessive salivation (traditional)

Kaolin is a soft, white clay mineral that has long been used in antidiarrheal products and skincare because it absorbs water and binds substances in the gut. Older diarrhea products often combined it with pectin, but modern guidelines favor rehydration and other treatments, and high-quality evidence for kaolin alone is limited. It is generally considered low-risk for short-term use, but check with your pharmacist or doctor before use, especially during pregnancy or breastfeeding.
