Does it work?
Moderate evidence
Benfotiamine is rated possibly effective for diabetic neuropathy (nerve pain from diabetes). For diabetic nephropathy (kidney damage from diabetes), the evidence points to possibly ineffective.
For Alzheimer disease, alcohol use disorder, and peripheral neuropathy (nerve damage in the limbs), there isn't enough reliable evidence to rate whether it works.
How safe is it?
Well-documented data
Benfotiamine is generally well tolerated in studies, though long-term safety hasn't been fully worked out. In one clinical trial of 98 people taking 300–600 mg daily for six weeks, a small number reported allergic skin reactions and a small number reported mild stomach discomfort — rates no different from placebo.
There isn't enough safety data on pregnancy, so the evidence advises against use unless your doctor advises otherwise. Safety while breastfeeding hasn't been well studied, so caution is advised if you're nursing.

Benfotiamine is a more bioavailable and physiologically active derivative of Thiamine. Unlike regular thiamine, a water-soluble nutrient also known as Vitamin B-1, Benfotiamine is lipid (fat) soluble. Benfotiamine readily passes through intestinal mucosal cells, where it is converted into physiologically active thiamine.
