Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product is pure bilberry fruit extract powder—one active ingredient. Bilberry is a dark berry whose extract concentrates the fruit's natural compounds, chiefly anthocyanidins, which give it its deep color and are responsible for its effects on vision, clotting, and blood sugar.
There are no inactive ingredients listed for this powder.
Does it work?
Leans against
The evidence for bilberry extract is mixed and often thin. Night vision is rated possibly ineffective, meaning the research doesn't support a benefit there.
For age-related cognitive decline, age-related macular degeneration (AMD), atherosclerosis, high cholesterol (hyperlipidemia), and chronic venous insufficiency—a condition where leg veins don't return blood well—the evidence is insufficient to rate it one way or the other. In short, we don't yet have solid data showing this extract works for any of these uses.
How safe is it?
Well-documented data
Bilberry fruit and juice from food are generally well tolerated, but supplement safety data is limited and long-term safety isn't well established. The safety notes advise against concentrated supplement doses due to that lack of data.
The most common side effects are dark-colored stools, gas, and mild stomach discomfort—some trial participants also reported temporary GI upset. Your mouth, tongue, and teeth may temporarily darken from the pigment, though this isn't harmful.
In pregnancy and lactation, bilberry is rated likely safe, so food amounts are not a concern. For supplement doses during pregnancy or nursing, talk with your doctor or pharmacist for personalized guidance, since the safety data for concentrated supplements in these situations is limited.
Meds to double-check
Moderate interaction found
Before taking bilberry extract, check with your doctor or pharmacist if you take blood thinners or antiplatelet drugs (such as warfarin or aspirin)—bilberry may raise bleeding risk. Also check if you're on diabetes medications, cancer drugs like erlotinib, or any medication broken down by the liver enzyme CYP2E1.
No interactions are documented for any ingredients we could not check, because this product contains only one ingredient that was reviewed.
The bottom line
Scorecard at a glanceFully disclosed formula with graded evidence leaning against its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
If you're interested in bilberry for vision or general health, keep in mind the evidence is weak or absent for the conditions it's marketed for. The bigger priority is checking whether it's safe alongside your medications—especially blood thinners, diabetes drugs, or erlotinib.
Talk with your doctor or pharmacist before you start, and use the interaction checker on this page for your exact medication list.

HelloPharmacist Interaction Report
Bilberry Extract Powder from BulkSupplements.com contains one active ingredient—bilberry fruit extract—that interacts with several medication types.
The most serious concern is with blood thinners and antiplatelet drugs (medications that prevent clots): bilberry may increase bleeding risk when combined with these, because research shows its active compounds can slow down platelet clotting. This is rated Moderate severity.
Read the full breakdown — every affected drug type, severity by severity
Bilberry also carries Moderate-severity interactions with diabetes medications. It may lower blood sugar on its own, which could push your levels too low if you're already taking drugs to control diabetes.
Additionally, it may affect how your body processes certain cancer drugs—specifically erlotinib (Tarceva)—and it can alter the activity of a liver enzyme (CYP2E1) that breaks down other medications, though the real-world impact of that enzyme change remains unclear.
Bilberry fruit extract is the only ingredient we checked. Altogether, these interactions span 275 individual medications.

For each ingredient in Bilberry Extract Powder with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Bilberry Fruit Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, bilberry fruit extract might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro, animal, and clinical research suggest that anthocyanidin extracts from bilberry can inhibit platelet aggregation.
Antidiabetes Drugs
Theoretically, bilberry leaf or fruit extract may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal research suggests that bilberry leaf extract might have blood glucose-lowering activity. Also, one small clinical trial in patients with type 2 diabetes shows that taking bilberry fruit extract 470 mg as a single dose prior to an oral glucose tolerance test lowers plasma glucose levels when compared with placebo.
Cytochrome P450 2E1 (Cyp2E1) Substrates
Theoretically, bilberry fruit extract might decrease levels of drugs metabolized by CYP2E1.
Animal research shows that exposure to small concentrations of bilberry extract in drinking water for around one month increased CYP2E1 activity by 31%. However, exposure over a 2-month period did not increase CYP2E1 activity. This effect has not been reported in humans.
Erlotinib (Tarceva)
Theoretically, bilberry fruit extract might reduce the efficacy of erlotinib.
In vitro research suggests that bilberry fruit extract and its constituents, delphinidin and delphinidin-3-O-glucoside, inhibit the activity of erlotinib. This interaction has not been reported in humans.
