Peony is generally well tolerated for short-term use when taken by mouth, though high-quality safety data are limited. The most common side effects are mild and gastrointestinal: abdominal distension, loss of appetite, diarrhea, nausea, and general stomach discomfort.

In one clinical trial, diarrhea occurred in 5% of patients taking a peony preparation at a higher dose for 24 weeks. There is also a case report of bleeding (gum bleeding, nosebleeds, skin bruising) in someone on warfarin who switched peony-containing herbal products.

During pregnancy, the safety data advises against use — it's rated possibly unsafe, so talk with your doctor or pharmacist before using it if you're pregnant or planning to become pregnant. Anticoagulant/Antiplatelet Drugs

Theoretically, combining peony with anticoagulant or antiplatelet drugs might increase the risk of bleeding.
In vitro research suggests that peony might have antiplatelet, anticoagulant, and antithrombotic effects.

However, preliminary research in humans shows that peony root does not alter levels of losartan (Cozaar), which is also metabolized by CYP2C9.
