Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: devil's claw extract, a plant-based substance used traditionally for joint and digestive support. The capsule also contains inactive ingredients — vegetable cellulose (the capsule material), rice flour, magnesium stearate, and silicon dioxide — which are fillers and binders that help hold the supplement together.
Does it work?
Moderate evidence
Devil's claw is rated possibly effective for back pain and osteoarthritis based on available evidence. For atherosclerosis, there isn't enough reliable evidence to rate how well it works.
If you're considering it for back pain or joint discomfort, talk with your doctor or pharmacist about whether it might fit into your care plan.
How safe is it?
Well-documented data
Devil's claw is generally well tolerated for short-term use in healthy adults, but it does carry some cautions. The most common side effects are digestive — allergic skin reactions, diarrhea, and indigestion — especially at higher doses.
Rarely, gastrointestinal bleeding has been reported. Some people have also experienced headache, dizziness, ringing in the ears, or loss of appetite, though it's unclear if devil's claw caused these.
Avoid devil's claw during pregnancy because it may affect the uterus and safety hasn't been established. Also avoid it while breastfeeding due to insufficient safety data.
If you're pregnant, planning pregnancy, or nursing, talk with your doctor or pharmacist before taking this product.
Meds to double-check
Moderate interaction found
Before taking devil's claw, double-check these medication types with your doctor or pharmacist: blood thinners like warfarin (Moderate risk), drugs processed by your liver through CYP2C19, CYP2C9, or CYP3A4 enzymes (Moderate risk), medications transported by P-glycoprotein (Minor risk), and acid-blocking drugs for heartburn — H2-blockers and proton pump inhibitors (Minor risk).
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Devil's Claw Extract may help with back pain or osteoarthritis, but it interacts with blood thinners, many medications processed by your liver, and acid-blocking drugs. If you take warfarin or any prescription medication, check with your doctor or pharmacist before starting it.
It's not for pregnancy or breastfeeding. Side effects are usually mild and digestive, but rarely gastrointestinal bleeding occurs.

Devil's Claw Extract by The Vitamin Shoppe Drug Interactions
HelloPharmacist Interaction Report
Devil's Claw Extract by The Vitamin Shoppe contains one active ingredient — devil's claw — which interacts with several medication categories.
The most serious concern is with blood thinners like warfarin (Coumadin): devil's claw may increase warfarin's activity, raising the risk of bleeding. In one case, a person taking both developed purpura (bruising), though it's unclear whether devil's claw alone was responsible.
Read the full breakdown — every affected drug type, severity by severity
Devil's claw may also affect how your body processes a broad range of prescription drugs. It appears to inhibit three major liver enzymes (CYP2C19, CYP2C9, and CYP3A4) based on lab studies, which could theoretically raise blood levels of medications metabolized by these pathways — though this hasn't been confirmed in people yet.
Additionally, it may interfere with P-glycoprotein, a transport system that moves drugs out of cells, potentially increasing levels of affected medications.
Finally, devil's claw has been reported to increase stomach acid, which could reduce how well acid-blocking medications work — both H2-blockers and proton pump inhibitors (PPIs) used to manage heartburn and reflux. Altogether, these interactions span 805 individual medications.
Before you start devil's claw, check your exact prescription and over-the-counter medications with the search tool on this page.

Devil’s Claw extract
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, devil's claw might increase levels of drugs metabolized by CYP2C19.
In vitro research shows that devil's claw might inhibit CYP2C19, although this has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, devil's claw might increase levels of drugs metabolized by CYP2C9.
In vitro research shows that devil's claw might inhibit CYP2C9, although this has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, devil's claw might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that devil's claw might inhibit CYP3A4, although this has not been reported in humans.
Warfarin (Coumadin)
Theoretically, Devil's claw might increase the activity of warfarin.
In one case report, purpura occurred in a patient taking warfarin and devil's claw concurrently. This might indicate over-anticoagulation. It is unclear if this was due to Devil's claw or other contributing factors.
H2-Blockers
Theoretically, devil's claw might decrease the effectiveness of H2-blockers.
Devil's claw has been reported to increase stomach acid, which might interfere with the effects of H2-blockers.
P-Glycoprotein Substrates
Theoretically, devil's claw might increase levels of P-glycoprotein substrates.
In vitro research shows that devil's claw inhibits P-glycoprotein, which transports many drugs out of cells. This might increase intracellular levels of P-glycoprotein substrates, although it is unclear if this effect would be clinically important.
Proton Pump Inhibitors (Ppis)
Theoretically, devil's claw might decrease the effectiveness of PPIs.
Devil's claw has been reported to increase stomach acid, which might interfere with the effects of PPIs.
