It has documented interactions with three medication types, all moderate severity. The most serious concern is with sedatives and other CNS depressants — poria mushroom extract has sedative properties in animal studies, and theoretically could enhance both the intended and unwanted effects of these drugs, though this hasn't been proven in humans yet.

Poria mushroom may also interfere with anticholinergic drugs (medications that reduce nerve signaling). Animal research suggests the extract reduces an enzyme that breaks down acetylcholine, which could theoretically weaken these drugs' effects.
Similarly, it might add to the effects of cholinergic drugs (medications that boost the same nerve pathway). Again, these are theoretical concerns based on animal work — human evidence is lacking.
