The evidence for lactoferrin is mixed and depends on what you're using it for. It's rated possibly effective for sepsis and for preventing pregnancy-related iron deficiency.

Lactoferrin is generally well tolerated at typical doses, though long-term safety data in children are limited. The most common side effects at standard doses are diarrhea and constipation.
Higher doses (7.2 grams daily) increase the risk of side effects and may cause fatigue, chills, loss of appetite, and nausea. In rare cases, newborns in trials have reported jaundice, bloody stool, bowel obstruction, and intestinal perforation.
