It's traditionally been used to stimulate menstruation and may affect the uterus — avoid it if you're pregnant. While breastfeeding, safety is unknown and madder's pigments can pass into breast milk, so it's best avoided then too.
If you do use it, expect red-colored urine, saliva, and sweat — that's the madder's pigment at work and is harmless on its own, but be aware it can stain contact lenses. Contact dermatitis (skin irritation) has been reported in people who handled madder directly.
