Protease and papain are generally well tolerated when used short-term at normal doses, though both can trigger allergic reactions in sensitive people — itching, watery eyes, sneezing, stomach cramps, or diarrhea. High doses of papain may be unsafe and could cause severe stomach inflammation or, rarely, esophageal perforation.

Occupational exposure to airborne enzyme dust can cause respiratory allergies including wheezing and asthma, though that's less of a concern with a chewable tablet. During pregnancy, concentrated papain and unripe papaya products are best avoided due to safety concerns.
